What does a clinical documentation reviewer do?
A clinical documentation reviewer checks the note for what is missing or wrong before it becomes a billing or audit problem, and queries the clinician for what the record does not support. With AI drafting more notes, the job has shifted from writing them to judging what an AI draft got wrong.

Common questions
Is AI replacing medical scribes?
Copy-type scribing is going away, because ambient AI drafts the note faster. What has not gone away is someone competent judging whether that draft is accurate and complete, which is the surviving version of the job.
What is a compliant query?
A question to the clinician that presents the clinical indicators found in the record and asks for clarification without suggesting the answer or the code that would pay more. Leading queries are a compliance problem.
