- A measure needs a numerator, a denominator, and an exclusion rule.
- Approve or refer. This desk does not issue the denial.
- Concurrent looks while care is underway. Retrospective looks after.
- Hospital-acquired conditions are safety and payment events.
- A copied medication list is not a reconciliation.

- Applying an exclusion so the rate looks better.
- Skipping Study in Plan-Do-Study-Act.
- Quiet deletion of an incident report.
- Closing a HEDIS gap with a screening that never happened.
