The rule
Assess, plan with named owners, coordinate the handoff, and evaluate the goal. Coverage doubt is referred, not denied from this desk.
If the next owner, the medication list, or the follow-up time is missing, the transition is incomplete.
Never miss
- CCMC's current blueprint has six domains, including reimbursement and ethics.
- Social determinants decide whether the plan can be carried out.
- Autonomy stands. A competent refusal still needs a safer alternative plan.
- Outcomes are measured against written goals, not call volume.
- Dual loyalty is named, not hidden.
- Psychotherapy notes have a higher confidentiality bar.
Traps
- A hope in place of a receiving site that has accepted the patient.
- Picking the shorter stay to please a payer when clinicians conflict.
- Labeling a patient noncompliant and stopping options.
- Discussing the case by name in an elevator.
Shortcuts
