What does a remote medical scribe do?

A medical scribe writes the clinician's note in the electronic health record during or just after a visit: the history, exam findings, orders and plan, for the clinician to review and sign. A remote, or virtual, scribe does the same work over a secure audio or video link. More and more, the job is checking an AI-drafted note rather than typing it.

Also advertised as: virtual medical scribe, remote scribe, clinical scribe, documentation assistant.

What a medical scribe does day to day

Here is what a remote medical scribe does between clock-in and clock-out.

  • Prepare the chart before clinic: pull the history, recent results and open problems for each visit
  • Listen to the visit and document the history, review of systems, exam findings and plan in the EHR
  • Queue orders, referrals and follow-ups for the clinician to sign, within what the employer allows
  • Check notes drafted by ambient AI or speech recognition for errors, gaps and things that were never said
  • Close each note quickly so the clinician can review and sign it the same day
  • Handle every record under HIPAA privacy rules and the employer's security policy

The clinician owns the note and signs it. A scribe records what the clinician finds and decides, and does not diagnose, advise patients or sign anything.

Medical scribe vs medical transcriptionist vs documentation specialist

RoleWhen the work happensWhat the role does
Medical scribe (this page)During or just after the visitWrites the clinician's note in the EHR for the clinician to sign
Medical transcriptionistAfter the visit, from a recordingTurns dictation into formal reports and edits speech-recognition drafts for accuracy (BLS)
Clinical documentation specialistAfter the note is writtenReviews notes and AI drafts for gaps and queries the clinician about what the record does not support

Skills and credentials for medical scribes

  • Medical terminology, anatomy basics and common abbreviations
  • Fast, accurate typing and comfort moving around an electronic health record
  • Careful listening, including accents and fast speech
  • HIPAA privacy habits: minimum necessary access, secure devices, no records on personal accounts

Most scribe jobs do not require a credential. For the related transcription and documentation work, the Association for Healthcare Documentation Integrity issues the Registered Healthcare Documentation Specialist (RHDS) credential, which it says is valid for 3 years (checked 19 October 2026). Read postings before paying for any exam. Other credentials are compared on the certifications page.

How do you become a remote medical scribe?

  1. Learn the language of the note. Medical terminology and the parts of a visit note: history, review of systems, exam, assessment and plan. Nurses already have most of this.
  2. Learn the privacy rules. The medical virtual assistant training page covers HIPAA basics for remote work.
  3. Practice in an EHR. Work through charts on the free Simulated EHR Workspace. Every case is fictional and holds no real patient data. For the product names postings use, read the list of EHR and EMR systems.
  4. Read real postings. See remote medical scribe jobs to see the hours, location rules and tools employers name.
  5. Apply with proof of how you work. Start with the medical VA resume guide. No course or app can promise a hire, and employers set pay.

Nurses: clinical knowledge makes the notes faster and more accurate, but scribe work does not use your nursing license. If you want review work that does, compare clinical documentation review and utilization review.

In the Philippines or Latin America: remote scribe work reaches candidates through healthcare outsourcing firms and medical VA agencies, and each posting sets its own location and hours. Read the Philippines starting guide, the Latin America page and the agency directory.

How much do medical scribes make?

The US Bureau of Labor Statistics does not publish a separate figure for medical scribes. For medical transcriptionists, the closest occupation it tracks, it reports a US median annual wage of $40,410 ($19.43 an hour) in May 2025 (BLS Occupational Outlook Handbook, checked 19 October 2026). That is transcriptionist pay in the US, not scribe pay, and not pay for remote work from another country.

Scribe pay is set by each employer and country. We do not publish a figure without a primary source.

Is medical scribing still a career with AI?

Typing the note word for word is fading, because ambient AI and speech recognition draft it faster. BLS expects medical transcriptionist jobs to fall 4 percent from 2025 to 2035 for that reason (BLS). What remains is checking whether the draft is accurate and complete, which is why scribe skills lead naturally into clinical documentation review.

Common questions

What does a medical scribe do?

A medical scribe documents a patient visit in the electronic health record for a clinician: the history, exam, orders and plan. The clinician stays responsible for the note and signs it. A remote scribe does this from another location over a secure connection.

What is the meaning of medical scribe?

A medical scribe is a documentation assistant who writes the clinician's notes so the clinician can focus on the patient. The scribe records what the clinician finds and decides. The scribe does not make clinical decisions.

Do you need a certification to be a medical scribe?

Usually not. Many employers train scribes on the job and ask for medical terminology, typing accuracy and EHR comfort instead. Credentials exist for nearby documentation work, such as the Registered Healthcare Documentation Specialist (RHDS) from the Association for Healthcare Documentation Integrity, but check each posting before paying for one.

What is the difference between a medical scribe and a medical transcriptionist?

A scribe documents the visit as it happens or right after, working alongside one clinician. A medical transcriptionist turns recorded dictation into a formal report later, and today often edits a speech-recognition draft. Both need medical terminology and careful listening.

Can you be a remote medical scribe from the Philippines or Latin America?

Some employers and healthcare outsourcing firms hire remote scribes outside the US, and each posting states the location, hours and pay it accepts. Strong English, medical terminology and US time-zone availability matter most. RemoteNurse does not place scribes or promise a hire.
