The rule
Confirm the measure year, the denominator, and any exclusion before you outreach. Do not invent a completed screening.
A confirmed open gap goes to the outreach path. Closure needs the source the specification accepts.
Never miss
- Numerator, denominator, exclusion.
- A claim without a matching note is not always enough.
- Hard-to-reach is not an exclusion.
- Last year is not automatically this year.
- The first line of a case note is the one question the reader has to answer today.
- A note that stands alone attaches each fact to the document you read it in, ends with a named owner and the one next action, and writes an unsettled question down instead of resolving it in silence.
Traps
- Entering a screening that never happened.
- Excluding a member because the rate is ugly.
- Closing on a rumor.
