Never miss

  • Numerator, denominator, exclusion.
  • A claim without a matching note is not always enough.
  • Hard-to-reach is not an exclusion.
  • Last year is not automatically this year.
  • The first line of a case note is the one question the reader has to answer today.
  • A note that stands alone attaches each fact to the document you read it in, ends with a named owner and the one next action, and writes an unsettled question down instead of resolving it in silence.

Traps

  • Entering a screening that never happened.
  • Excluding a member because the rate is ugly.
  • Closing on a rumor.

Shortcuts

  • Specification, then the record, then outreach.
  • Preferred channel.
  • Dated outreach.

Terms

  • Measure year. The period the specification uses. Confirm it before outreach.
  • Exclusion. A documented reason the member is not in the measure. It lives in the spec and the record.
  • Source-backed closure. The result sits in the place the specification accepts.
  • Standing note. Attach each fact to the document you read it in. End with a named owner. Leave an unsettled question on the page.
