Never miss
- Numerator, denominator, exclusion.
- A claim without a matching note is not always enough.
- Hard-to-reach is not an exclusion.
- Last year is not automatically this year.
- The first line of a case note is the one question the reader has to answer today.
- A note that stands alone attaches each fact to the document you read it in, ends with a named owner and the one next action, and writes an unsettled question down instead of resolving it in silence.
Traps
- Entering a screening that never happened.
- Excluding a member because the rate is ugly.
- Closing on a rumor.

- Specification, then the record, then outreach.
- Preferred channel.
- Dated outreach.
Terms
- Measure year. The period the specification uses. Confirm it before outreach.
- Exclusion. A documented reason the member is not in the measure. It lives in the spec and the record.
- Source-backed closure. The result sits in the place the specification accepts.
- Standing note. Attach each fact to the document you read it in. End with a named owner. Leave an unsettled question on the page.
