An expired authorization or a purpose-less 'whole chart' request stops until authority is clear.
Never miss
- Payer claim review is payment, still limited.
- Psychotherapy notes have a higher bar.
- Do not invent a missing operative report to complete a packet.
- Letterhead is not automatic authority.
- Affirming that care was given as billed is the treating practitioner's signature, not the reviewer's note.
- You do not pick which of two conflicting entries holds unless the record denotes a correction.
- A retrospective review of a closed encounter is read against the claim date first.
Traps
- Personal Gmail because it is faster.
- Releasing more because the requester is loud.
- White-out on an expired date.
