Never miss

  • Payer claim review is payment, still limited.
  • Psychotherapy notes have a higher bar.
  • Do not invent a missing operative report to complete a packet.
  • Letterhead is not automatic authority.
  • Affirming that care was given as billed is the treating practitioner's signature, not the reviewer's note.
  • You do not pick which of two conflicting entries holds unless the record denotes a correction.
  • A retrospective review of a closed encounter is read against the claim date first.

Traps

  • Personal Gmail because it is faster.
  • Releasing more because the requester is loud.
  • White-out on an expired date.
