Never miss
- The filing window lives on the notice.
- There is no universal day count.
- A clinical appeal is signed by a clinician who knows the case.
- Timely-filing fights are won with proof of original submission.
Traps
- Changing dates so a denial expires.
- Adding a diagnosis the record never stated.
- Dumping the lifetime PDF with no pointer.
