Prior authorization, Philippines

What do you record on a prior authorization from the Philippines?

A nurse or medical VA in the Philippines can prepare a prior authorization: identifiers, dates, units, and the document that is missing. On 5 November 2026 two Philippines-listed postings asked for that paperwork and named no US nursing license. You record and escalate. You do not decide medical necessity.

Short answers

What does a prior authorization assistant in the Philippines record?
Identifiers, the service or medication name already on the form, dates, units, the portal status, and the field that is blank. The clinical reason stays with the US clinician who ordered the service.
Does a Philippine nursing license let you approve a US prior authorization?
No. A Philippine nursing license is not a US nursing license. The two postings read on 5 November 2026 do not say it authorizes a US clinical decision.
Were the two example postings still open on 5 November 2026?
Yes. Workable's public job record listed both as published that day. Roles close without notice, so open the page again before you apply.

What did two Philippines-listed prior authorization postings still say on 5 November 2026?

On 5 November 2026 both pages were still published. Winning Assistants lists the role in the Philippines, on US Central hours, and asks for hands-on medication prior authorization experience. GMVA lists a remote Philippines role, names night shifts, and asks for prior authorization or US healthcare BPO experience. Neither page names a US or Philippine nursing license.

The Philippines country page keeps the longer openings list. This guide uses two of those seats as examples of the task, re-opened on 5 November 2026. The hours, pay line, and clearance rule in the table are the words on each page. They are not a RemoteNurse offer, and a role can close without notice.

The figures at the bottom of this guide are US workload counts. They are not the pay, the hiring rate, or the hours on a Philippines posting.

Winning AssistantsMedication prior authorization for a US psychiatry and behavioral health practice. Hands-on medication prior authorization and insurance verification experience are required. The page does not print a US clock. The benefits line says "Starting rate is $800 (USD) monthly" and "Employment Type: Independent Contractor."

What do you record on a fictional prior authorization packet?

Use a made-up packet, never a real chart. Record the member id, the service or medication name as printed, the dates and units the form already shows, the portal status, and the field that is blank. Copy only what the form asks for. Leave clinical reasoning blank for the ordering clinician.

The packet below is fiction. Member Q-100, Northwind Family Practice, and Payer Example are not a real person, clinic, or plan. Nothing in it is a recommendation to start, stop, or change a medicine.

  • Received: the date and time the portal shows, in the timezone the screen prints
  • Member Q-100, as printed. Do not add an identifier from a different screen
  • Medication line: copy the name already on the form. Do not choose a different drug
  • Quantity and days: copy them if the form already shows them. If the field is blank, write "blank"
  • Portal status: pending. Record the reference number the portal shows, or "none shown"
  • Missing: the diagnosis-code field the form asks for is empty

What do you escalate to a US clinician instead of deciding?

Send the blank-field list to the US clinician who ordered the service. Do not write that the care is medically necessary, sign as the prescriber, or tell the patient the plan approved or denied it. For a Medicare Advantage plan, an adverse medical-necessity decision has to be reviewed by a physician or other appropriate professional before the plan issues it. A Philippine license is not that role.

A Philippine nursing license is not a US nursing license. The two postings do not say it qualifies you for a US clinical decision. If the portal later shows a denial, record the reason text it shows and the item still missing. Preparing that note is not an appeal decision.

For Medicare Advantage, 42 CFR 422.566(d), read 5 November 2026, says that if the plan expects to issue a partially or fully adverse medical-necessity organization determination, a physician or other appropriate health care professional must review it before the plan issues the decision. That rule is about the plan's determination. It is not a grant of that review to a Philippines-based assistant.

What privacy limit applies when you copy from the chart?

A covered entity or business associate must limit protected health information to the minimum necessary for the task, with stated exceptions such as a provider's treatment request. Copy the fields the form needs. Do not paste an unrelated note into the packet because it might help.

The employer's procedure names the fields for this desk. The rule does not give you a personal right to open every part of the chart. Winning Assistants' benefits block says it provides HIPAA training for contractors who handle information for US clients. That training is the employer's, not a license.

The minimum-necessary standard is 45 CFR 164.502(b), read 5 November 2026. It has stated exceptions, including a disclosure to or request by a health care provider for treatment.

Which prior authorization clock can you write on the packet?

The CMS-0057-F fact sheet limits its decision times, 72 hours for an expedited request and seven calendar days for a standard one, and its specific denial reason, to non-drug items and services. They apply at impacted payers, excluding qualified health plan issuers on the Federally Facilitated Exchanges, starting January 1, 2026. They are already in effect. A medication request cannot take its deadline from them.

The fact sheet says that, as with all policies in the final rule, the denial-reason provision does not apply to prior authorization decisions for drugs. The Winning Assistants posting and the fictional packet are medication prior authorization, so they cannot use this rule to infer a deadline. Use the employer or that payer's procedure. For a non-drug request, still do not stamp this maximum on a commercial clinic portal unless the employer says that payer is in scope. If the screen shows no received time, record that gap instead of inventing one.

The decision maximums are on the CMS-0057-F fact sheet, read 5 November 2026: impacted payers, excluding QHP issuers on the FFEs, send prior authorization decisions within 72 hours for expedited requests and seven calendar days for standard requests. Beginning in 2026 they must give a specific reason for a denial. The fact sheet says that, as with all policies in the final rule, this provision does not apply to prior authorization decisions for drugs. Those operational policies have a compliance date starting January 1, 2026, so on 5 November 2026 they are already in effect, and only for non-drug items and services. The medication posting and the fictional packet cannot use them to set a deadline. Use the employer or payer procedure.

Frequently asked questions

Is the pay on those postings a salary RemoteNurse promises?
No. The table quotes the pay line each page prints. RemoteNurse does not set pay, place candidates, or promise that the role stays open.
Where is the free prior authorization course?
On the prior authorization desk. That page is the course. This guide is the record-and-escalate example for someone working from the Philippines.
Can the medication example use the CMS 72-hour or seven-day clock?
No. Those decision times and the specific denial reason apply only to non-drug items and services at impacted payers, excluding QHP issuers on the Federally Facilitated Exchanges, from January 1, 2026. The medication example and the Winning Assistants posting use the employer or payer procedure instead.
