If the written rule is silent, ask the named owner. If identity collides, add a third identifier.
Never miss
- Interpreter access is a safety issue.
- Same-name patients need date of birth plus medical record number.
- Personal email is not a records channel.
- A subpoena goes to HIM or privacy, not out the front door.
- No-shows are documented and recovered, not deleted.
- Clearinghouse rejects are often demographic. Fix them fast.
Traps
- Looking up a neighbor 'just to help.'
- Interpreting an MRI because the patient asked.
- Double-booking a procedure without prep.
- Attaching a lab to the chart that opened first.
