The Accredited Case Manager (ACM) is built for case management inside the health delivery system and across care transitions. If your target work is acute-care case management, discharge planning, or transitions of care, it speaks directly to that setting.

What the ACM is
The ACM is administered by the American Case Management Association (ACMA). It is a health-delivery-system case-management credential, offered in a nurse track (ACM-RN) and a separate social-work track (ACM-SW), and it centers on the hospital and transitions-of-care work that sits next to utilization review.
Who it is for
Registered nurses working in acute-care case management, discharge planning, and transitions of care who want a delivery-system-focused credential to back the workflow judgment they already practice. Qualifying social workers pursue the separate ACM-SW track.
Requirements
- An active, unrestricted RN license (the ACM is also offered to qualifying social workers in a separate ACM-SW track).
- Case-management experience in a health delivery system; confirm the current amount required with ACMA.
- Confirm current eligibility, application steps, and the exam format with the American Case Management Association before applying.
