For clinics hiring a medical VA
Should a clinic hire a medical VA from the Philippines or Latin America?
Hire from Latin America if patients need Spanish or you want an assistant on your own daytime hours; hire from the Philippines if English-only work on a shift that is overnight in Manila suits you. Published rates overlap, from Hello Rache's $9.50 an hour (Philippines) to MedicalVirtual's $8 to $12 (Latin America); MEDVA and Ask Velma recruit in both, and RemoteNurse is free medical VA training, not a staffing agency.
Philippines vs Latin America at a glance
| Factor | Philippines | Latin America |
|---|---|---|
| US clinic hours | A US day shift is overnight in Manila (9 pm to 5 am or 10 pm to 6 am for New York's 9 to 5). Providers say their assistants work your hours. | Bogotá, Mexico City and São Paulo sit within 0 to 2 hours of New York, so your day is their day. |
| Spanish for patients | No Spanish option on the Philippines-only provider page we read (Hello Rache). Test it in the interview if you need it. | MedicalVirtual offers bilingual English-Spanish assistants; Ask Velma lists a Bilingual Virtual Medical Assistant role; Virtual Latinos offers English plus Spanish or Portuguese. |
| Published rates (read Oct 26, 2026) | Hello Rache: $9.50 an hour flat, no contracts, no setup fees. Ask Velma (both regions): $10 an hour for 90 days, then $10.50, minimum 32 hours a week. | MedicalVirtual: $8 to $12 an hour. Virtual Latinos: $1.6K to $4.4K a month by experience level. Ask Velma: same rate card as above. |
| Who recruits there | Hello Rache (Philippines), MEDVA and Ask Velma (both regions). | MedicalVirtual and Virtual Latinos (Latin America), MEDVA and Ask Velma (both regions). |
| BAA in writing | Ask Velma says a BAA is in place from day one. We found no BAA statement on Hello Rache's home page; ask for one. | MedicalVirtual says it signs a BAA before patient-data access. We found no HIPAA or BAA statement on Virtual Latinos' pricing page; ask. |
| HIPAA | Same rules either way: your practice stays responsible for its safeguards, and HHS says offshore risk belongs in your risk analysis. | Same rules either way: a BAA, Security Rule safeguards and minimum-necessary access, set by your practice. |
What does a medical VA cost from each region?
Only some providers publish a rate. These are the rates on their own pages on Oct 26, 2026; confirm them on a written quote, because prices and terms change.
| Provider | Where its assistants are | Published rate | Terms it states |
|---|---|---|---|
| Hello Rache | Philippines | $9.50/hour flat | No long-term contracts, no setup fees, cancel anytime. |
| Ask Velma | Philippines and Latin America | $10/hour for the first 90 days, $10.50 after | Minimum 32 hours a week, billed every two weeks; no setup, recruiting or cancellation fees; BAA from day one. |
| MedicalVirtual | Latin America | $8 to $12/hour | HIPAA training before the start and a signed BAA before patient-data access; English or English-Spanish. |
| Virtual Latinos | Latin America | $1.6K to $4.4K/month (Starter $1.6K to $2.9K, Professional $1.7K to $3.4K, Advanced $2.1K to $4.4K) | No recruitment fee; covers remote roles in general, not healthcare only. |
| MEDVA | Philippines and Latin America | Not published on the pages we read | Ask for a written quote. |
On these numbers the regions overlap, so price alone does not settle the choice. We found no reliable public salary dataset for medical VAs in the Philippines or Latin America, so this page quotes no worker pay; an agency rate also covers recruiting, payroll and support, not only the assistant’s pay. For a US baseline, the BLS median for medical secretaries and administrative assistants was $45,930 a year in May 2025.
How much do US clinic hours overlap?
- Philippines: In Manila, 9:00 am to 5:00 pm in New York is 10:00 pm to 6:00 am the next day in January (US standard time) and 9:00 pm to 5:00 am the next day in July (US daylight saving time).IANA Time Zone Database
- Latin America: In Bogotá, 9:00 am to 5:00 pm in New York is 9:00 am to 5:00 pm in January (US standard time) and 8:00 am to 4:00 pm in July (US daylight saving time).IANA Time Zone Database
- Latin America: In Mexico City, 9:00 am to 5:00 pm in New York is 8:00 am to 4:00 pm in January (US standard time) and 7:00 am to 3:00 pm in July (US daylight saving time).IANA Time Zone Database

A Philippines-based assistant can cover your full day, but as a night shift. Hello Rache and MedicalVirtual both say their assistants work your US hours. Ask who covers holidays and absences, and whether the schedule is fixed in the contract.
Do your patients need Spanish?
About 44.9 million people age 5 and older spoke Spanish at home in the US in 2024, roughly 1 in 7, and 58.9% of them said they speak English very well.
If a real share of your calls, reminders or intake happens in Spanish, Latin America is the simpler fit: MedicalVirtual offers bilingual English-Spanish assistants, Ask Velma lists a bilingual role, and Virtual Latinos offers English plus Spanish or Portuguese. For English-only phone and back-office work, both regions can fit. Whichever you pick, test the language you need in a live mock patient call before you sign.
HIPAA steps before an offshore assistant sees patient data
The rules are the same for Manila and Bogotá. Your practice stays responsible for its safeguards.
- Sign a Business Associate Agreement before any patient data moves. HHS says the HIPAA Rules let a covered entity disclose PHI to a business associate only with satisfactory assurances, in a contract or other written arrangement (a BAA), that the business associate will safeguard it. An agency whose assistants handle your patients' records on your behalf is generally a business associate, and HHS says a business associate needs its own BAA with any subcontractor before sharing PHI. HHS publishes sample BAA provisions you can compare a provider's draft against.
- Put the assistant inside your Security Rule safeguards. HHS's Security Rule summary lists administrative, physical and technical safeguards: a risk analysis, a named security official, workforce training, access control, audit controls, authentication and transmission security. Give the assistant a unique login on your systems, keep patient data in those systems, and review access logs.
- Give only the minimum necessary access. The Privacy Rule's minimum necessary standard requires covered entities to limit uses and disclosures of, and requests for, PHI to the minimum needed for the purpose. In practice, a scheduling assistant should not see full charts; set role-based access before day one.
- Weigh the location in your risk analysis. HHS's cloud guidance says the HIPAA Rules have no requirements specific to ePHI handled by a business associate outside the US, but that offshore risks can vary by location and should be taken into account in the Security Rule risk analysis. That applies to Manila and Bogotá alike.
- Check the training, then add your own procedures. HHS's Security Rule summary says a regulated entity, which includes a business associate, must train all workforce members on its security policies and procedures. Ask the provider how and when it trains, then walk the assistant through your practice's own procedures; HHS's training page links free material.
When neither makes sense, and when to hire locally
- The work is in person: rooming patients, vitals, front-desk check-in or anything hands-on. A remote assistant cannot do it.
- The task needs a US clinical license, such as clinical judgment by a nurse. Check the license rule for that desk before you hire anywhere.
- Your practice cannot yet give a remote worker a unique login, role-based access and audit logs, or no one owns the BAA and the risk analysis. Fix that first; the HIPAA steps above apply to any remote hire.
- You need more than one assistant can cover, or nobody has time to supervise. A managed agency or a local hire may fit better than a single remote assistant.
- Cost is the only reason, but patients want someone in the room. The BLS median for US medical secretaries and administrative assistants was $45,930 a year in May 2025 (receptionists $38,010). That is pay before benefits and payroll costs; weigh it against a written remote quote for the same hours rather than assuming offshore always wins.
Not sure which desk the work is? Compare the remote healthcare desks and their license rules first.
Which should you choose?
- Spanish-speaking patients or live daytime calls: Latin America.
- English-only back office on a fixed US shift: either region; compare written quotes.
- One provider for both regions: MEDVA or Ask Velma recruit in both.
- A BAA in writing from day one: ask every provider; Ask Velma and MedicalVirtual say they sign one.
- You want to hire directly and train the assistant yourself: post the role on RemoteNurse and point candidates to the free medical VA courses.
Candidate-side guides: medical VA work from the Philippines and medical VA work from Latin America. More providers compared: MEDVA alternatives.
Common questions
Should a clinic hire a medical VA from the Philippines or Latin America?
Decide on language and hours first. Latin America fits clinics that need Spanish for patients or an assistant on a normal local workday close to US time. The Philippines fits English-only work on an agreed shift, which is overnight in Manila. Published rates overlap: Hello Rache lists $9.50 an hour (Philippines) and MedicalVirtual $8 to $12 (Latin America), read October 26, 2026.
Is a Latin American medical VA cheaper than a Filipino one?
Not on the published rates we read on October 26, 2026. Hello Rache lists $9.50 an hour for Philippines-based assistants, Ask Velma lists $10 an hour for its first 90 days in both regions, and MedicalVirtual lists $8 to $12 an hour in Latin America. We found no reliable public salary dataset for medical VAs in either region, so compare written quotes for the same hours and role.
What are US clinic hours in Manila and Bogotá?
New York's 9:00 am to 5:00 pm is 10:00 pm to 6:00 am the next day in Manila in January and 9:00 pm to 5:00 am the next day in July. In Bogotá it is 9:00 am to 5:00 pm in January and 8:00 am to 4:00 pm in July, because Colombia does not change its clocks.
How many US patients speak Spanish at home?
About 44.9 million people age 5 and older spoke Spanish at home in the US in 2024, roughly 1 in 7, and 58.9% of them said they speak English very well, according to Census data summarized by USAFacts. If many of your patients call in Spanish, a bilingual assistant from Latin America is the simpler fit.
Do I need a BAA to hire an offshore medical VA?
If the agency or contractor will create, receive, maintain or transmit your patients' PHI on your behalf, yes: HHS says a covered entity needs a business associate agreement giving satisfactory assurances before it shares PHI with a business associate. Ask Velma and MedicalVirtual say they sign one; ask any other provider in writing.
Does HIPAA allow patient data to be handled outside the US?
HHS's cloud guidance says the HIPAA Rules have no requirements specific to ePHI handled by a business associate outside the US, but that the risks can vary by location and should be weighed in your Security Rule risk analysis. Keep data in your own systems and give the assistant only the minimum necessary access.
When should a clinic hire locally instead?
When the work is hands-on or in person, needs a US clinical license, or your practice cannot yet give a remote worker controlled, logged access. The BLS median pay for US medical secretaries and administrative assistants was $45,930 a year in May 2025, which you can weigh against a remote quote.
Is RemoteNurse a staffing agency?
No. RemoteNurse (remotenurse.org) is free medical VA training and a job board. It does not place, employ, pay or manage assistants, or sign a BAA with your practice. You can post a role and hire directly, or use one of the agencies above. RemoteNurse is a separate company from Remote Nurse Connection.
