A matching refill is confirmation, not a new prescription. Dose changes and new indications go to the clinician.

Never miss

• Allergy conflicts are safety events.
• Controlled-substance exceptions follow the written protocol.
• A gap in therapy needs an urgent clinical path plus the missing PA element.
• You do not invent the indication.

Traps

• Doubling a dose because the patient asked.
• Using a neighbor's leftover pills.
• A fake approval number.

Shortcuts

• Drug, dose, directions, last fill, remaining refills, active order.
